Southern Cross Care (Tas.) Inc. has 40 Community Aged Care Packages (pdf) in the Hobart metropolitan area from Claremont to Sandy Bay and the Eastern Shore. In addition there are 4 Extended Aged Care at Home and 4 Extended Aged Care at Home Dementia Packages.
The aim of these packages is to establish a program of care in partnership with the client which enables and encourages independence and quality of life. Flexibility is the key to the package of care and the agreed range of services can change as the clients care needs change. Eligibility for these packages is after assessment by the Aged Care Assessment Team (link). The client contribution is modest and no one is refused care because of financial hardship.
All field staff are qualified and skilled in the delivery of flexible care which is tailored to meet each clients needs in an individual and supportive way. Professional and experienced case management ensures seamless care which is tailored to meet each individual’s changing needs. The packages are designed to maintain and restore independence and quality of life.
Services include but are not limited to:
For those clients who have been assessed by the Aged Care Assessment Team as eligible to receive an Exteneded Aged Care at Home or Extended Care At Home Dementia Package (pdf), Southern Cross Care (Tas.) Inc. can assist with the provision of integrated high level care in the city of Hobart.
These packages provide more complex care levels than Community Aged Care Packages and are designed for those people who would be eligible for High Care in a residential facility. Naturally the primary carer is closely involved with the case management in a true partnership to achieve optimal outcomes for client and carer alike. Innovation and flexibility are key factors in the design of the care package together with a personalised match of staff to client.
The services are varied and include but are not limited to
The staff have training in the special needs of those people living with high care and dementia and understand the specific needs of those who give the majority of the care to the client. A high level of professional case management is central to the care planning which is formulated in close consultation with all stakeholders.
Southern Cross Care (Tas.) Inc. is also a contracted provider for eligible Veterans under the Veterans Home Care Scheme. Assessment for eligibility is conducted by an external assessor who may be contacted by phoning 1300 550 450.
Commonwealth Funding is also provided for our unique In Home Diversional Therapy Programme (pdf). This service provides meaningful respite for the primary carer of someone living with dementia. Activities are designed to enhance the psychological, spiritual social and physical well being of the client by developing a unique plan of care for each person. All staff are qualified in this specialist area and have great skills in the care of people living with dementia and the special needs of those who care for them.
In addition to Government subsidised services we can also offer the same high standard of supportive care on a In Home Fee for Service (pdf) basis to those who for some reason are ineligible for other funding or who may require additional services.
